Please use the following steps when completing your forms online
In compliance with upcoming federal regulations, we are a paperless office.  You will be asked to electronically sign your forms when you visit our office for your appointment.

  • Click on the “Forms” button below
  • Type in today’s date in the appointment date space.
  • Fill out the 3 available forms: 1. Consent for Internet; 2. Medical & Dental History; 3.Patient Information.
  • Fill in all required information blanks.
  • You can use the arrows at the bottom of the forms to navigate from page to page.
  • When forms are complete, click “SUBMIT”.
  • You will be asked to sign your forms on our electronic signature pad, when you visit our office for your appointment.

Some forms will require review every 6-12 months.  If you would like to access your treatment/ account information online, you will need to register with our website.  Please feel free to do so at any time.
